Registration Information at Alma d'arte

 

In order to register here at Alma d'arte there are a few steps that must be completed. First, you need to come by the front office (located at 402 W. Court Ave) and pick up a Registration packet. Included in the packet are forms you must fill out such as Student Information, Medical Referral, etc. After you complete the application you need to bring the packet back to the office along with a number of other documents such as a copy of the students Birth Certificate, Transcripts from Previous School(s), etc. Below, you will find a list of all the forms and documents needed for registration.

  • Student Information Form
  • Home Language Survey
  • Authorization for Release of Information
  • Medical Referral Form
  • Family Information Form
  • Media Documentation Permission
  • Application for Free and Reduced Meals (if applicable.)
These Documents are NOT Provided in the Registration Packet
  • Copy of Birth Certificate
  • Copy of Social Security Card
  • Copy of IEP (if applicable.)
  • Transcripts from Previous School(s)
  • Complete Immunization Records
  • Examples of Art History
  • Letter(s) of Reference
  • Arts Narrative
